This PE questionnaire will help gain an insight into staff confidence levels in teaching, planning and assessing different activity areas within PE so that you can plan effective CPD and support in making improvements and ensuring high quality teaching and learning. The questionnaire also contains questions on health and getting the staff on board with a vision for PE, health and sport in your school. It is a word document that you can use as a template to alter as you need, add your own school logo on and shape the feedback you need.
